dc.description.abstractObjectives: To characterize the epidemiological profile and clinical manifestations of individuals with Helicobacter pylori infection in the gastric mucosa, to determine the prevalence of the pathogen’s virulence gene cagA, and to investigate associations between bacterial strains, epidemiological characteristics, and symptomatology. Methods: A cross-sectional study was conducted in Passo Fundo, south of Brazil. Adults aged ≥ 18 years of both sexes who underwent upper gastrointestinal endoscopy at a regional hospital and had H. pylori infection confirmed by histopathological analysis of gastric biopsy specimens were included. Participants completed an epidemiological questionnaire, and infected tissue samples were analyzed for the presence of H. pylori cagA gene using conventional polymerase chain reaction (PCR). Statistical analysis included frequency distribution of descriptive variables, prevalence estimates for gene positivity, and association analyses with 95% confidence intervals. Results: The final sample comprised 40 patients selected from 225 individuals, yielding an H. pylori infection prevalence of 17.8% in the study population. The sample was predominantly female (70.0%), aged ≥ 50 years (65.0%), residing in urban areas (87.5%), living with ≥ 3 household members (52.5%), with current or previous tobacco use (52.5%) and alcohol consumption (70.0%). All participants were symptomatic, with heartburn (77.5%) and abdominal and epigastric pain (62.5% each) being the most prevalent complaints. Molecular analysis revealed that 52.5% of participants were positive for the cagA gene. There was no statistically significant relation between H. pylori strains that were positive or negative for the cagA gene and the clinical-epidemiological factors evaluated. Conclusion: This study demonstrated a low prevalence of H. pylori infection, consistent with the regional variability reported in the literature. The characterization of the sample and the frequency of cagA-positive strains were in agreement with current evidence. Despite the use of gold-standard molecular detection methods, the sampling strategy and relatively small sample size may have limited the statistical power of the study. Considering the sample and the methodology employed, it can be concluded that the presence of the cagA virulence gene in the gastric mucosa is not related with the clinical and epidemiological profile of infected individuals.pt_BR
dc.description.resumoObjetivos: Analisar o perfil epidemiológico e as manifestações clínicas dos indivíduos com infecção por Helicobacter pylori na mucosa gástrica, além de avaliar a prevalência do gene de virulência cagA do patógeno e relacionar as cepas bacterianas com a epidemiologia e com a sintomatologia da amostra; Métodos: Estudo transversal realizado em Passo Fundo - RS, Brasil. Incluíram-se os participantes de ambos os sexos e idade igual ou superior a 18 anos, que realizaram endoscopia digestiva alta em um hospital regional e cuja análise histopatológica de biópsia gástrica constatou infecção por H. pylori. Aplicou-se um questionário epidemiológico e as amostras teciduais infectadas foram analisadas para a presença do gene cagA do H. pylori pela técnica convencional de PCR. A análise estatística consistiu na distribuição de frequências das variáveis descritivas, na prevalência da positividade do gene e na relação obtida entre essas, com intervalo de confiança de 95%. Resultados: A amostra foi de 40 pacientes selecionados do total de 225 a partir da positividade para a infecção, que teve prevalência de 17,8%. Epidemiologicamente, houve destaque para o sexo feminino (70,0%), idade ≥ 50 anos (65,0%), residência em zona urbana (87,5%) com ≥ 3 moradores (52,5%), consumo ativo ou pregresso de tabaco (52,5%) e de álcool (70,0%). Clinicamente, toda a amostra era sintomática, com maior prevalência de pirose (77,5%) e de dores abdominais e epigástricas (62,5% cada). Na análise molecular, 20,0% era portador do gene cagA. Não houve relação estatisticamente significativa entre as cepas do H. pylori positivas ou negativas para o cagA e os fatores clínico-epidemiológicos. Conclusão: O estudo demonstrou baixa prevalência da infecção por H. pylori, conforme a variabilidade regional da literatura. A caracterização da amostra e a frequência de cepas cagA apresentaram-se de acordo com evidências atuais. Apesar da detecção molecular padrão-ouro, a amostragem e a reduzida amostra podem ter limitado o poder estatístico. Considerando a amostra e a metodologia empregada, conclui-se que a presença do gene de virulência cagA na mucosa gástrica não se relacionou com o perfil clínico-epidemiológico dos infectados.pt_BR
