Title: Análise espaço-temporal do HIV no Estado de Santa Catarina: 2008-2017

Resume: A presente tese foi desenvolvida com o objetivo geral de analisar a distribuição espaçotemporal do HIV no Estado de Santa Catarina, Brasil, no período 2008-2017. Para tanto, realizou um estudo longitudinal retrospectivo, com análises no nível individual e de municípios e, portanto, também ecológico, a partir de dados de casos de HIV e de aids provenientes dos sistemas de informação em saúde de vigilância de casos (SINAN), de mortalidade (SIM) e de monitoramento laboratorial (SISCEL). Os dados foram compilados através de relacionamento probabilístico, com registros vinculados por um identificador pessoal único, para a identificação da totalidade de casos. Como resultados foram identificados 67.340 registros: 29.734 (44,2%) pelo SINAN, 5.540 (8,2%) pelo SIM e 32.066 (47,6%) pelo SISCEL. Após o relacionamento de bancos, o tamanho da população de HIV foi de 45.707 indivíduos com HIV no estado, no período. O primeiro objetivo específico foi estimar o número de indivíduos com idade entre 15 e 79 anos com HIV em Santa Catarina, Brasil, no período de 2008 a 2017, agregando aos casos identificados, os possíveis casos subnotificados. Estimativas de captura-recaptura foram obtidas por modelagem log-linear com os principais efeitos e interação entre os sistemas de informação, ajustados por idade, sexo e período. Um ajuste para a subnotificação de mortes relacionadas à aids utilizou dados publicados sobre causas mal definidas de morte, e mortalidade por causa relacionada ao HIV. Neste estudo, foram agregados 44 indivíduos pela captura-recaptura, e 1.512 mortes não relatadas relacionadas à doença pelo HIV, estimando total de 47.263 indivíduos infectados pelo HIV (IC 95% 47.245-47.282) e incidência correspondente de 93 (IC de 95% 91-96) p/100 mil em Santa Catarina. A apuração de casos de 62,9%, 78,5% e 67,8% foi estimada para SINAN, SIM e SISCEL, respectivamente. A partir dos casos identificados pelos sistemas (45.707 casos), taxas de incidência segundo subgrupos de análise orientaram a descrição da distribuição espaço-temporal da incidência do HIV em Santa Catarina, Brasil, 2008-2017, segundo objetivo especifico desta tese. Para tanto, os casos foram geograficamente referenciados segundo municípios de residência, e ajustados por modelo estatístico de Poisson que considerou aspectos individuais, socioeconômicos e demográficos no nível municipal. A análise estatística identificou a alta incidência do HIV em Santa Catarina, com maior taxa em homens entre 25-54 anos em todos os períodos analisados. Destaque para o aumento significativo em homens jovens (15-24 anos) no último período, em todas as macrorregiões de saúde. A distribuição espacial da incidência por município e período se mostrou heterogênea, com maiores incidências nos municípios do litoral catarinense. A identificação dos efeitos de fatores individuais, socioeconômicos e demográficos no nível municipal, na incidência do HIV foi o terceiro objetivo específico deste trabalho. Este, foi conduzido em acordo a um estudo ecológico, com abordagem espaço-temporal. Regressão de Poisson multivariada e multinível identificou os efeitos fixos e randômicos dos fatores em análise na incidência do HIV. Para análise espacial, as taxas foram suavizadas pelo estimador bayesiano empírico local. Índices Moran global e local identificaram a dependência e a autocorrelação espacial local. O estudo reafirmou o sexo masculino e faixa etária de 25-54 anos como fatores de risco para o HIV, assim como o grau de urbanização, a densidade demográfica, o IDH do município, a taxa de desemprego e a macrorregião de residência. A análise da autocorrelação espacial revelou aglomerados espaciais de alta incidência localizados principalmente na região litorânea, em todos os períodos. Considerados os achados conclui-se que: diante da subnotificação dos sistemas de informação identificada, o relacionamento de bancos para vinculação de dados deve ser rotina na vigilância em saúde no nível estadual; diante da persistência da alta incidência do HIV no estado, e da tendência de aumento principalmente entre os homens jovens das macrorregiões litorâneas, devem ser reorientadas e reforçadas as ações de prevenção nesta população. Ainda, ao identificar que a maior variação da incidência do HIV dada pelos efeitos randômicos (por outros efeitos relevantes, porém não disponíveisno banco de dados) ocorre no nível municipal, sugere-se o desenvolvimento pesquisas futuras de identificação de outros fatores de nível municipal relacionados à incidência do HIV.
Abstract: The objective of this work was to analyze the spatio-temporal distribution of HIV in the State of Santa Catarina over the 2008-2017 period. To this end, a retrospective longitudinal study was carried out, using the data at both individual and municipal (ecological) levels. Individual data on HIV and AIDS cases were retrieved from the epidemiological surveillance (SINAN), mortality (SIM), and laboratory monitoring (SISCEL). The data were compiled through a probabilistic algorithm whereby the records were linked by a unique personal identifier to identify all cases. As a result, 67.340 records were identified: 29.734 (44.2%) by SINAN, 5.540 (8.2%) by SIM and 32.066 (47.6%) by SISCEL. After merging the data sources, the size of the HIV-infected population was estimated at 45.707 over the period analyzed. This was the first step towards the first specific objective of the study - to estimate the number of HIV-infected individuals aged 15 to 79 years in the state over this period. Capture-recapture estimates were obtained by log-linear modeling with the main effects and interaction between information systems, adjusted for age, sex, and period. Also, an adjustment for underreporting of AIDS-related deaths used published data on ill-defined causes of AIDS death and mortality. In this study, 44 individuals were aggregated by capture-recapture and 1.512 unreported deaths related to AIDS, estimating a total of 47.263 HIV-infected individuals (95% CI 47.245-47.282) and the corresponding incidence of 93 (95% CI 91-96) per 100.000 in Santa Catarina. The coverage of 62.9%, 78.5%, and 67.8% was estimated for SINAN, SIM, and SISCEL, respectively. The second specific objective of this work was to estimate the HIV incidence rate for sex-byage population groups on the municipal level and analyze their spatial and temporal distribution of HIV incidence in the state over the 2008-2017 period. All 47.505 cases registered/identified in the aforementioned analysis were geographically referenced to the municipalities of residence and smoothed by Poisson regression with random effects. Further statistical analysis identified a high incidence of HIV in Santa Catarina, particularly in men between 25 and 54 years of age, in all periods analyzed. There was a significant increase in young men (15-24 years) in the 2013-2017 period in all state regions. The municipal spatial distribution of the HIV incidence by period was heterogeneous, with greater incidence in the coastal municipalities. The third specific objective of this study was to identify the effects of individual, socioeconomic and demographic factors at the municipal level on the HIV incidence - an ecological study with space-time approach. To this end, multivariate multilevel Poisson regression with both fixed and random effects was used. For spatial analysis, the rates were smoothed by the local empirical Bayesian estimator. Global and local Moran indices identified dependence and local spatial autocorrelation. The study reaffirmed the male gender and the age group between 25-54 years old as risk factors for HIV, as well as the degree of urbanization, the demographic density, the Human Development Index, the unemployment rate, and the macro-region of residence. The analysis of the spatial autocorrelation revealed spatial clusters of high incidence located mainly in the coastal region of the state, over the periods analyzed. Overall, the present study results underline the following points: a) given the size of underreporting identified for the health information systems, their linkage should be routine, b) persistence of high HIV incidence in the state and its increase among young men in the coastal regions calls for reinforcement and reorientation of the HIV prevention efforts. Also, the finding that the greatest variation in the incidence of HIV came from random effects (i.e. other relevant effects not available in the database) at the municipal level, points to the need for future research to identify additional factors that influence the incidence at this level.
