Título: As repercussões da pandemia da unidade de terapia intensiva adulta geral a partir da criação da unidade específica para covid-19 na instituição hospitalar

Resumo: Diante do registro da disseminação da Coronavirus disease 2019 (COVID-19) no mundo, a Organização Mundial da Saúde, em março de 2020, classificou a COVID-19 como uma pandemia. Concomitantemente, no Brasil identificou-se um rápido aumento na demanda por serviços de saúde, principalmente por leitos hospitalares em Unidade de Terapia Intensiva (UTI), constatando uma situação crítica do sistema de saúde para atender à demanda potencial gerada pela pandemia. Diante disto, várias regiões do Brasil trabalharam na abertura de UTIs exclusivas para pacientes com COVID-19. Assim, emergiu a pergunta de pesquisa: “Quais as repercussões da pandemia na UTI Adulto Geral, a partir da criação da UTI específica para COVID-19 na instituição hospitalar?” O objetivo geral desta pesquisa é, portanto, compreender as repercussões da pandemia na UTI Adulto Geral, a partir da criação da UTI específica para COVID-19 na instituição hospitalar. Trata-se de um estudo qualitativo do tipo pesquisa ação participante, desenvolvido com o método de Investigação do Itinerário de Pesquisa inspirado em Paulo Freire. A coleta ocorreu na forma de Círculos de Cultura Virtual (CCV) com dois enfermeiros assistenciais de instituições hospitalares do estado de Santa Catarina. O itinerário de Freire prevê três etapas: Investigação Temática, Codificação e Descodificação e Desvelamento Crítico. O processo analítico de investigação foi realizado durante todo o processo e com todos os participantes do CCV, realizado no dia 23 de março de 2021. Assim, a análise ocorreu concomitantemente com a investigação para determinar o desvelamento crítico das temáticas eleitas e discutidas com a literatura pertinente ao referencial teórico e metodológico. A pesquisa apresenta parecer favorável do Comitê de Ética em Pesquisa com Seres Humanos (CAAE: 4.068.387), atendendo aos critérios estabelecidos pela Resolução CNS n°466/2012. Dentre as repercussões geradas na UTI Adulto Geral, foram percebidos pontos negativos, tais como o sofrimento, a sobrecarga física e mental, o medo, a raiva, a ansiedade e a tristeza dos profissionais de saúde, e pontos positivos, tais como a união da enfermagem, a valorização de si mesmos e da unidade em que trabalham, a reformulação da assistência e a resiliência psicológica. A pandemia da COVID- 19 mostra-se como um momento inédito na história do Sistema Único de Saúde e caracterizase atualmente como um cenário de incertezas e desafios. A utilização do CCV demonstrou-se uma importante ferramenta para o alcance dos resultados, pois possibilitou mergulhar no universo dos participantes, cenário central da pesquisa. Além de elucidar as repercussões da pandemia nas UTIs Adulto Geral, também ofereceu benefícios aos sujeitos envolvidos.Sugere-se, portanto, que espaços de escuta e acolhimento sejam oportunizados aos profissionais que atuam direta e indiretamente na pandemia da COVID-19.
Abstract/Resumen: In view of the record of the spread of Coronavirus disease 2019 (COVID-19) in the world, the World Health Organization, in March 2020, classified COVID-19 as a pandemic. Concomitantly, in Brazil a rapid increase in the demand for health services was identified, mainly for hospital beds in the Intensive Care Unit (ICU), confirming a critical situation in the health system to meet the potential demand generated by the pandemic. In view of this, several regions of Brazil have worked on opening exclusive ICUs for patients with COVID- 19. Thus, the research question emerged: "What are the repercussions of the pandemic in the General Adult ICU, from the creation of the specific ICU for COVID-19 in the hospital?" The general objective of this research is, therefore, to understand the repercussions of the pandemic in the General Adult ICU, from the creation of the specific ICU for COVID-19 in the hospital. This is a qualitative study of the type of participatory action research, developed with the Research Itinerary Research method inspired by Paulo Freire. The collection took place in the form of Virtual Culture Circles (CCV) with two nursing assistants from hospital institutions in the state of Santa Catarina. Freire's itinerary includes three stages: Thematic Investigation, Codification and Decoding and Critical Unveiling. The analytical investigation process was carried out throughout the process and with all participants of the CCV, held on March 23, 2021. Thus, the analysis occurred concurrently with the investigation to determine the critical unveiling of the themes chosen and discussed with the literature pertinent to the theoretical and methodological framework. The research presents a favorable opinion from the Ethics Committee in Research with Human Beings (CAAE: 4,068,387), meeting the criteria established by CNS Resolution No. 466/2012. Among the repercussions generated in the Adult General ICU, negative points were perceived, such as the suffering, physical and mental overload, fear, anger, anxiety and sadness of health professionals, and positive points, such as the union of nursing, valuing themselves and the unit in which they work, reformulating care and psychological resilience. The COVID-19 pandemic shows itself as an unprecedented moment in the history of the Sistema Único de Saúde and is currently characterized as a scenario of uncertainties and challenges. The use of the CCV proved to be an important tool to achieve the results, as it made it possible to dive into the universe of the participants, the central scenario of the research. In addition to elucidating the repercussions of the pandemic in the Adult General ICUs, it also offered benefits to the subjects involved. Itis suggested, therefore, that listening and welcoming spaces are offered to professionals who work directly and indirectly in the pandemic of COVID-19.
