Title: Análise do tempo para início de tratamento oncológico de neoplasia maligna do encéfalo no Brasil entre 2015 e 2024

Resume: As neoplasias malignas do encéfalo constituem um importante problema de saúde pública devido à elevada morbimortalidade, rápida progressão clínica e impacto funcional significativo. O início oportuno do tratamento é fundamental para reduzir a progressão tumoral e ampliar as possibilidades terapêuticas, sendo também um indicador da qualidade da assistência oncológica. No Brasil, desigualdades regionais e limitações estruturais podem influenciar o acesso ao tratamento dentro do prazo, de até 60 dias, estabelecido pela Lei nº 12.732/2012. Nesse cenário, este estudo buscou responder a seguinte pergunta de pesquisa: Existem fatores sociodemográficos, regionais e terapêuticos associados ao início oportuno do tratamento oncológico em pacientes adultos com neoplasia maligna do encéfalo no Brasil? Nesse contexto, o presente estudo objetivou avaliar o tempo para início do tratamento oncológico em pacientes adultos com neoplasia maligna do encéfalo no Brasil, considerando características epidemiológicas, regionais e terapêuticas. Trata-se de um estudo observacional, retrospectivo, transversal, com abordagem quali-quantitativa, realizado a partir de dados secundários do Painel-Oncologia/DATASUS. Foram analisados registros de pacientes adultos diagnosticados com neoplasia maligna do encéfalo (CID-10 C71) entre janeiro de 2015 e dezembro de 2024. O desfecho foi o tempo entre o diagnóstico e o início do tratamento, categorizado em ≤60 dias e >60 dias. As associações entre as variáveis foram avaliadas por meio do teste do Qui-quadrado de Pearson e dos modelos de riscos proporcionais de Cox (p-valor < 0,05). Foram analisados 26.380 casos de neoplasia maligna do encéfalo, dos quais 58,2% iniciaram o tratamento em até 60 dias após o diagnóstico. Houve associação significativa entre o início oportuno do tratamento e as variáveis faixa etária, região geográfica e modalidade terapêutica (p<0,001), enquanto o sexo não apresentou associação na análise geral. O tratamento cirúrgico apresentou a maior proporção de início oportuno (96,7%), seguido da combinação quimioterapia e radioterapia (47,1%), quimioterapia isolada (44,8%) e radioterapia isolada (39,4%). Na análise multivariada, observou-se associação entre fatores regionais e o início oportuno dos tratamentos cirúrgico, quimioterápico e radioterápico, enquanto idade e sexo apresentaram influência variável conforme a modalidade terapêutica. Para a associação entre quimioterapia e radioterapia não foram identificados fatores associados ao tempo de início do tratamento. Os resultados evidenciam que, mesmo após mais de uma década da implementação da Lei dos 60 dias, parcela expressiva dos pacientes ainda enfrenta atrasos para o início do tratamento oncológico. As diferenças observadas entre regiões sugerem a influência de desigualdades na distribuição de serviços especializados e infraestrutura assistencial no país. Além disso, a associação entre idade, modalidade terapêutica e tempo de início do tratamento reforça a necessidade de estratégias específicas para ampliar a equidade e a eficiência do cuidado em neuro-oncologia. O início oportuno do tratamento oncológico em pacientes com neoplasia maligna do encéfalo no Brasil esteve associado a fatores regionais, etários e terapêuticos, evidenciando a persistência de desigualdades assistenciais e a necessidade de fortalecimento das políticas públicas voltadas à efetivação do acesso ao tratamento dentro do prazo legal.
Abstract: Malignant neoplasms of the brain constitute a major public health problem due to high morbidity and mortality, rapid clinical progression, and significant functional impact. Timely initiation of treatment is crucial to reducing tumor progression and expanding therapeutic options, and is also an indicator of the quality of cancer care. In Brazil, regional inequalities and structural limitations can affect access to treatment within the 60-day timeframe established by Law No. 12.732/2012. In this context, this study sought to answer the following research question: Are there sociodemographic, regional, and therapeutic factors associated with the timely initiation of cancer treatment in adult patients with brain cancer in Brazil? In this context, the present study aimed to evaluate the time to initiation of cancer treatment in adult patients with malignant brain neoplasms in Brazil, considering epidemiological, regional, and therapeutic characteristics. This is an observational, retrospective, cross-sectional study with a mixed-methods approach (qualitative and quantitative), conducted using secondary data from the Oncology Panel/DATASUS. Records of adult patients diagnosed with malignant neoplasm of the brain (ICD-10 C71) between January 2015 and December 2024 were analyzed. The outcome was the time from diagnosis to the start of treatment, categorized as ≤60 days or>60 days. Associations between variables were assessed using Pearson's chi-square test and Cox proportional hazards models (p-value < 0.05). 26,380 cases of malignant brain neoplasia were analyzed, of which 58.2% started treatment within 60 days after diagnosis. There was a significant association between the timely start of treatment and the variables age group, geographic region, and therapeutic modality (p<0.001), while gender showed no association in the general analysis. Surgical treatment had the highest proportion of timely initiation (96.7%), followed by the combination of chemotherapy and radiotherapy (47.1%), chemotherapy alone (44.8%), and radiotherapy alone (39.4%). In the multivariate analysis, an association was observed between regional factors and the timely initiation of surgical, chemotherapeutic, and radiotherapeutic treatments, whereas age and sex showed variable influence by therapeutic modality. No factors associated with the timing of treatment initiation were identified for the combination of chemotherapy and radiotherapy. The results show that, even more than a decade after the implementation of the 60-day law, a significant portion of patients still face delays in starting cancer treatment. The differences observed between regions suggest the influence of inequalities in the distribution of specialized services and healthcare infrastructure in the country. Furthermore, the association among age, therapeutic modality, and timing of treatment initiation underscores the need for targeted strategies to improve equity and efficiency in neuro-oncology care. The timely initiation of cancer treatment in patients with malignant brain neoplasms in Brazil was associated with regional, age, and therapeutic factors, highlighting the persistence of inequalities in care and the need to strengthen public policies aimed at ensuring access to treatment within the legal timeframe.
