Título : Fatores associados ao conhecimento e consumo de alimentos integrais por adultos e idosos

Resume: Introdução: O grão de cereal íntegro possui como estrutura o endosperma, o germe e o farelo. Quando o grão passa pelo processo de refino, grande parte do farelo e do germe são retirados, ocasionando perdas de diversos compostos, como vitaminas, minerais, fibras alimentares, lignanas, entre outros. Os produtos integrais são definidos como o alimento produzido com grãos integrais que não passaram pelo processo de refinamento. Devido a esse fato, esses alimentos possuem um maior aporte de micronutrientes, o que os torna mais saudáveis, diminuindo o risco de desenvolvimento de patologias e melhorando a qualidade de vida dos indivíduos. Apesar do consumo desses produtos ter aumentado substancialmente ao longo dos anos, grande parte da população ainda não faz uso desses alimentos. Objetivos: Os objetivos destes trabalhos foram: 1- Revisar a literatura sobre os aspectos associados ao consumo de grãos integrais; 2- Avaliar os fatores associados ao conhecimento e consumo de alimentos integrais por adultos e idosos no sudoeste do Paraná. Métodos: No que diz respeito a metodologia empregada para o desenvolvimento da revisão de literatura narrativa, foram utilizadas as bases de dados Science Direct, Scielo, Pub Med, sendo empregadas as seguintes palavras-chave: whole grain, whole foods, fiber content, no período de 2000 a 2017. Para a seleção dos artigos, foi realizada a leitura do resumo dos mesmos. Quando, através do resumo, percebia-se que o artigo se enquadrava no assunto esperado, esse era lido na íntegra e suas partes mais relevantes foram inseridas na revisão de literatura. Em relação ao artigo original, os participantes da pesquisa foram abordados aleatoriamente durante os meses de novembro de 2017 a janeiro de 2018, em locais de ampla circulação de pessoas na cidade de Francisco Beltrão - PR. Realizou-se uma entrevista composta por 41 questões objetivas referentes ao perfil socioeconômico e ao conhecimento/consumo de produtos integrais. Os dados foram analisados por meio de estatística descritiva e submetidos ao teste de qui-quadrado e regressão logística univariada e multivariada pelo programa estatístico Stata versão 13. Resultados e discussão: No que diz respeito a revisão de literatura, foi visto que os grãos integrais e os alimentos oriundos desses apresentam diversos benefícios ao funcionamento do organismo e podem reduzir o risco de diversas doenças crônicas não transmissíveis. Além disso, foi percebido que atualmente a população esta se preocupando cada vez mais com sua saúde e esses alimentos estão sendo bastante procurados, porém ainda uma grande parcela da população não os conhece e/ou não os consome, sendo necessárias intervenções tanto do poder público, como das indústrias alimentícias e profissionais da saúde para elevação do consumo e alcance dos níveis considerados ideais preconizados pelos órgãos de saúde. Também foi visto que há uma escassez de legislação, principalmente no Brasil, quando se fala em ―alimentos integrais‖, não possuindo rotulagem padrão nem definição especifica da quantidade de grãos integrais para esse tipo de produto, gerando muita confusão na hora dos consumidores realizarem a escolha no momento da compra. Outro ponto analisado foram as barreiras de consumo desses produtos, e as que mais apareceram nos estudos foram: hábito, os aspectos sensoriais do produto, custo, e preferência familiar. Em relação ao artigo original, foram entrevistadas 397 pessoas sendo a maioria do sexo feminino, raça branca, residentes na área urbana, com mais de 11 anos de escolaridade e pertencentes a classe socioeconômica B2- C1. Mais da metade dos indivíduos (54,6%) responderam que consumiam alimentos integrais, no entanto, 21,23% foram classificados como consumidores irregulares. Além disso, indivíduos do sexo feminino (OR: 2,24), ativos fisicamente (OR: 3,33), pertencentes a classe A-B1 (OR: 1,00), e com maior nível de conhecimento (OR: 1,66) demonstraram maior probabilidade de consumo dos mesmos. Constatou-se também que as pessoas que presumiam que esses alimentos deviam ter fibras alimentares, grãos integrais/não serem processados e outros componentes além dos citados, possuíam maiores chances (OR: 2,87/2,73/3,37, respectivamente) de consumo quanto a quem não sabia do que se tratava os alimentos integrais No que se refere ao consumo regular desses alimentos, os indivíduos com excesso de peso (OR: 0,45) apresentaram menores chances de ingestão. Conclusão: Através da revisão de literatura foi percebido que os alimentos feitos com os grãos integrais ocupam um lugar muito importante no mercado alimentício, pois apresentam vários benefícios a saúde associados ao seu consumo, porém ainda não atinge níveis considerados ideais de consumo. Também é percebido que há uma escassez de legislação referente a esse tema, principalmente no Brasil, sendo necessário estabelecimento de legislações efetivas que definam as quantidades de grãos integrais, de modo a possibilitar maior informação para a população. Já no que diz respeito ao artigo original, foi observado que os indivíduos do sexo feminino (OR: 2,24), ativos fisicamente (OR: 3,33), da classe A-B1 (OR: 1,00), e que possuíam maior conhecimento sobre alimentos integrais (OR:1,66) possuíam maiores chances de consumo dos mesmos. No que se refere ao consumo regular desses alimentos, os indivíduos com excesso de peso (OR:0,45) apresentaram menores chances de ingestão. Verificou-se também que os meios de comunicação e os profissionais de saúde (OR: 2,78) apresentam influencia no consumo regular dos alimentos integrais. Diante disso, percebe-se que a compreensão dos fatores associados ao conhecimento e ao consumo de alimentos integrais são de extrema importância para futuras intervenções em públicos alvos específicos.
Resumen : Introduction: The intact cereal grain has as structure the endosperm, the germ and the bran. When the grain goes through the refining process, much of the bran and germ are removed, causing loss of various compounds, such as vitamins, minerals, dietary fibers, lignans, among others. Whole-grain products are defined as food produced from whole grains that have not undergone the refinement process. Due to this fact, these foods have a higher intake of micronutrients, which makes them healthier, reducing the risk of developing pathologies and improving the quality of life of individuals. Although consumption of these products has increased substantially over the years, much of the population still does not use these foods. Objectives: The objectives of this work were: 1- Review the literature on the aspects associated to the consumption of whole grains; 2- Evaluate the factors associated with the knowledge and consumption of whole foods by adults and the elderly in southwestern Paraná. Methods: With regard to the methodology used for the development of the narrative literature review, we used the databases Science Direct, Scielo, Pub Med, using the following keywords: whole grain, whole foods, fiber content, no period from 2000 to 2017. For the selection of articles, a summary of the articles was read. When, through the abstract, it was perceived that the article fit the expected subject, it was read in its entirety and its most relevant parts were inserted in the literature review. Regarding the original article, the participants were randomly approached during the months of November 2017 to January 2018, in places of wide circulation of people in the city of Francisco Beltrão - PR. An interview was made with 41 objective questions regarding the socioeconomic profile and knowledge / consumption of whole products. Data were analyzed through descriptive statistics and submitted to the chi-square test and univariate and multivariate logistic regression using Stata statistical software version 13. Results and discussion: Regarding the literature review, it was seen that whole grains and foods derived from them have several benefits to the functioning of the organism and may reduce the risk of several chronic non-transmissible diseases. In addition, it was perceived that the population is becoming more and more preoccupied with their health and these foods are being much sought after, but still a large part of the population does not know them and / or does not consume them, being necessary interventions of both public power , and from the food industry and health professionals to increase consumption and reach the levels considered ideal recommended by health agencies. It was also seen that there is a shortage of legislation, especially in Brazil, when it refers to "whole foods", not having standard labeling or specific definition of the quantity of whole grains for this type of product, generating much confusion when consumers choice at the time of purchase. Another point analyzed was the consumption barriers of these products, and the ones that appeared most in the studies were: habit, the sensorial aspects of the product, cost, and family preference. Regarding the original article, 397 people were interviewed, the majority of whom were female, white, residents of the urban area, with more than 11 years of schooling and belonging to socioeconomic class B2-C1. More than half of the individuals (54.6%) answered that they consumed whole foods, however, 21.23% were classified as irregular consumers. In addition, female subjects (OR: 2.24), physically active (OR: 3.33), belonging to class A-B1 (OR: 1.00), and with a higher level of knowledge (OR: 66) were more likely to be consumed. It was also found that people who assumed that these foods should have dietary fiber, whole grains / not processed and other components besides those mentioned had higher odds (OR: 2.87 / 2.73 / 3.37, respectively) As for the regular consumption of these foods, individuals with excess weight (OR: 0.45) presented lower chances of ingestion. Conclusion: Through the literature review it was noticed that foods made with whole grains occupy a very important place in the food market, since they present several health benefits associated to their consumption, but still do not reach levels considered ideal consumption. It is also perceived that there is a shortage of legislation regarding this subject, mainly in Brazil, and it is necessary to establish effective legislation that defines the quantities of whole grains, in order to allow more information for the population. Regarding the original article, it was observed that female individuals (OR: 2.24), physically active (OR: 3.33), of class A-B1 (OR: 1.00), and that had greater knowledge about whole foods (OR: 1.66) had higher chances of consuming them. With regard to the regular consumption of these foods, individuals with excess weight (OR: 0.45) presented lower chances of ingestion. It was also found that the media and health professionals (OR: 2.78) have an influence on the regular consumption of whole foods. Therefore, it is understood that the understanding of the factors associated with knowledge and consumption of whole foods is of extreme importance for future interventions in specific target audiences.
