Title: Puberdade e maternidade: a incontinência urinária na gestação em adolescentes

Resume: Introdução: A incontinência urinária, definida como qualquer perda involuntária de urina, compromete o bem-estar físico, social e emocional. Embora documentada em adultas, a população adolescente apresenta vulnerabilidades específicas devido à imaturidade biológica e ao contexto psicossocial da gestação. Objetivo: Analisar, através de uma revisão sistemática, a ocorrência e os determinantes da incontinência urinária em gestantes adolescentes, bem como suas repercussões físicas, emocionais e sociais. Metodologia: Trata-se de uma revisão sistemática elaborada conforme as diretrizes PRISMA e o manual da Joanna Briggs Institute (JBI), registrada na plataforma OSF. A busca foi realizada nas bases PubMed, BVS e SciELO, utilizando a estratégia PICO. Foram incluídos nove estudos observacionais primários publicados nos últimos 10 anos, os quais passaram por avaliação de risco de viés por meio de checklists específicos do JBI. Resultados: A prevalência de IU em gestantes adolescentes variou entre 50% e 60,1%, com predomínio da incontinência urinária de esforço. Os fatores de risco identificados incluíram a idade materna entre 10 e 14 anos, peso materno superior a 90 kg, constipação intestinal, histórico prévio de perdas urinárias e a realização de episiotomia. No âmbito psicossocial, observou-se que o constrangimento e a vergonha levam à omissão do sintoma e ao isolamento social. Conclusão: A IU é uma condição frequente e subnotificada em gestantes adolescentes, agravada pela falta de investigação ativa no pré-natal. Por isso, torna- se fundamental a implementação de protocolos de rastreio e estratégias preventivas para mitigar os impactos negativos na qualidade de vida dessa população.
Abstract: Introduction: Urinary incontinence, defined as any involuntary loss of urine, compromises physical, social, and emotional well-being. Although documented in adults, the adolescent population presents specific vulnerabilities due to biological immaturity and the psychosocial context of pregnancy. Objective: To analyze, through a systematic review, the occurrence and determinants of urinary incontinence in pregnant adolescents, as well as its physical, emotional, and social repercussions. Methods: This is a systematic review conducted according to the PRISMA guidelines and the Joanna Briggs Institute (JBI) manual, registered on the OSF platform. The search was performed in the PubMed, VHL (BVS), and SciELO databases using the PICO strategy. Nine primary observational studies published in the last 10 years were included and underwent risk of bias assessment through specific JBI checklists. Results: The prevalence of UI in pregnant adolescents ranged from 50% to 60.1%, with a predominance of stress urinary incontinence. Identified risk factors included maternal age between 10 and 14 years, maternal weight exceeding 90 kg, intestinal constipation, a prior history of urinary loss, and the performance of episiotomy. In the psychosocial sphere, it was observed that embarrassment and shame lead to symptom omission and social isolation. Conclusion: UI is a frequent and underreported condition in pregnant adolescents, exacerbated by the lack of active investigation during prenatal care. Therefore, the implementation of screening protocols and preventive strategies is essential to mitigate the negative impacts on this population's quality of life.
